The Lunaria Client Pathway


Phase 1: Navigating the Diagnosis & Initial Planning

  • Feeling stuck or overwhelmed? We are your GUIDE—your first anchor in the storm.

    What We Will Do:

    • Meet with you regularly or at a cadence that feels right to you.

    • Provide emotional support as you process your diagnosis. 

    • Provide Education about Your Illness & Prognosis: Demystify medical jargon, translate complex prognostic data, and bring all information together from specialists to create a cohesive understanding.

    • Medication Review: Proactively identify polypharmacy and potential drug-drug interactions.

    • Help you identify and reach out to your support system/community and identify your core medical team.

    • Initial Spiritual Screening to understand your personal sources of strength: Are there any spiritual or religious practices that are important to your care?

  • Feeling like your feet are back under you and you’re ready to move forward? We are your GUIDE in building your path.

    What We Will Do:

    • Medical System Navigation: Help determine what medical appointments you need, how to keep them organized, and strategize scheduling.

    • Advance Care Planning: Facilitate deep discussions about what matters most and begin planning ahead for the future and create your Lunaria Narrative. 

    • Aligning Care with Beliefs: Ensure your medical decisions and Advance Care Planning documents align with your spiritual and cultural values.

    • Coordinate directly with your medical team as needed.

    • Map your support system: We help you identify your circle of support, define their specific roles (e.g., logistical, emotional, medical), plan easy ways to update them, and identify any crucial gaps that may exist.

    • Serve as a landing pad as you continue to process and adapt to a “new normal.”


Phase 2: Integrating Illness Into Life

  • Feeling like you’ve got a plan and you want to get back to living your life? We are your GUIDE in balancing priorities.

    What We Will Do:

    • Help you identify your values and your new medical responsibilities, guiding you to know when to focus on each—no more overwhelm.

    • Symptom Burden Assessment: Conduct routine, proactive screening for common symptoms (pain, fatigue, anxiety) and recommend interventions.

    • Functional Status Optimization: Assess and create plans (including recommendations for referrals) to maintain mobility, nutrition, and cognitive function.

    • Plan and prioritize health & wellness maintenance, including identifying and supporting necessary habit changes.

    • Help define a minimum acceptable quality of life and facilitate prioritization of meaningful activities and abilities. 

    • Referral to spiritual counselors, chaplains, or your own faith/spiritual community when requested.

  • Ready to organize your future to ensure clarity for yourself and your loved ones? We are your GUIDE to practical completion.

    What We Will Do:

    • Warm hand-off/referral to estate planning lawyers and financial planners.

    • POLST Discussion & Completion: Facilitate and complete state-specific medical forms based on your values and preferences outlined in your Lunaria Narrative (Washington State only). 

    • Help with funeral and after-death arrangements, including beginning Vigil Planning.

    • Offer a Practical Affairs Toolkit to organize passwords, key documents, and personal final wishes.

  • Ready to reflect on your impact and intentionally plan what you want to leave the world? We are your GUIDE in celebrating your story.

    What We Will Do:

    • Plan and create memory work through: writing, video, and/or visual art.

    • Help you tell your story and articulate your values.

    • Facilitate the creation of a Legacy Letter  (values, lessons, and hopes for future generations).

    • Guide the creation of letters to your loved ones for milestones after death.

    • Plan for your Virtual/Digital Legacy.

Phase 3: Transitions & End-of-Life

  • Feeling uncertain about health, illness, or treatment changes? We are your GUIDE to recalibration.

    What We Will Do:

    • Help identify triggers to know when you need to readdress aspects of your pathway.

    • Coordinate with your medical team and loved ones.

    • Readdress the Lunaria Narrative and advance care planning as needed.

    • Create Crisis Prevention Planning to manage decline and avoid unnecessary hospital visits.

  • When living as long as possible is no longer the primary goal, we are your GUIDE to maximized quality of life and comfort.

    What We Will Do:

    • Support you to decide when to stop disease-directed treatment(s).

    • Ensure you have the medical care you need at the end of your life—this may include hospice as well as other complementary modalities.

    • Help you live life to the fullest with comfort and adequate symptom management to optimize quality of life.

    • Provide you and your support system with education about your illness trajectory, signs/symptoms of end of life, and what to expect.

  • In your final hours, days, and weeks, we are your GUIDE in creating a space of comfort and meaning.

    What We Will Do:

    • Expert  Symptom Management : Provide recommendations to the hospice/medical team regarding refractory symptoms (pain, delirium).

    • Witness and hold space for bodily changes and ease the uncertainty through education.

    • Implement the Vigil Plan (creating the desired environment with music, readings, scent, etc.).

    • Guide loved ones through Rituals and Ceremony for closure.

    • Offer respite resources and practical caregiver support.

  • We are still your GUIDE in loss, offering support and structure in the immediate and long-term aftermath.

    What We Will Do:

    • Provide ongoing support for your loved ones in their grief.

    • Help identify funeral and after-care needs.

    • Immediate Post-Death Support: Guide the family through the necessary practical steps (contacting the funeral home, documentation).

    • Complicated Grief Screening: Provide education on normal grief and screen for indicators requiring referral to specialized support.

    • Support loved ones in Grief Integration, helping them find meaningful ways to incorporate the loss into their lives.
